Mississippi Medicaid expansion efforts stall despite potential benefits

Mississippi has not expanded Medicaid since the option became available in 2014, despite the potential to receive more than $2 billion annually from the federal government in recent years, according to sources familiar with the issue.

Support for expansion peaked during the 2024 legislative session, aided by then-House Speaker Jason White. However, efforts to pass expansion legislation failed after the passage of President Donald Trump’s so-called One Big Beautiful Bill, or HR 1, this summer. The bill, which included reductions to health care and safety net programs, removed financial incentives that had encouraged some states, including Mississippi, to expand Medicaid.

Forty states and the District of Columbia have expanded Medicaid, but none have done so since 2023. The bill was supported by Mississippi Republican U.S. Sens. Roger Wicker and Cindy Hyde-Smith. Legislation to expand Medicaid in Mississippi this session has been introduced but died without consideration by the Republican-led Legislature. Many health advocates in the state have remained largely silent on the issue.

A recent study by Families USA, a national nonprofit, estimates that Mississippi is leaving $73.8 million in federal funds on the table in 2026 by not expanding Medicaid. The report notes that thousands of Mississippians currently covered under traditional Medicaid could be eligible for expansion, which would increase federal contributions from 77.3% to 90% of costs. Moving these recipients to expanded Medicaid could save the state $25 million annually and generate $16.1 million in insurance premium tax revenue.

The study also highlights that other economic benefits, such as job growth, are projected by independent analyses, including the University Research Center. However, critics note that the state’s recent legislative priorities have focused on tax cuts and funding for private schools, rather than health care expansion. Despite federal restrictions, advocates remain hopeful that Mississippi can still pursue Medicaid expansion, citing potential health and financial benefits for the state’s residents.
